Please check each box when you complete it and return the signed form to Mrs. Kowalcyk to receive the registration links.
- See Ms. Dwyer to pick up form and schedule a meeting with Mrs. Broderick to discuss the available courses
- Attend the mandatory meeting to discuss the registration process (date TBD)
- Complete the Arkport Central School District Email Policy for ACE+ Students release form and return it to Mrs. Dwyer
- Complete the Certificate of Residency for the appropriate college based on your chosen course
- Join the online course Remind account using @arkonline.
- Sign and return this form to Mrs. Kowalcyk’s mailbox
